How do we know we are on track in achieving these healthcare and social goals? How do our partners and stakeholders know we are making time bound progress? SPM is a strategy that allows monitoring of progress made towards these goals and objectives. It allows for easy reporting to key stakeholders.

A workshop on SPM was conducted in Feb 2011 both in Nandyal and Satribari to introduce the concept of SPM and deriving hospital specific goals and objectives. Workshop was facilitated by Ms. Lyn Hill from 4B Serve and Mr. Calum Scott from Opportunity International Australia.

The Long wait for a reliable laboratory facility is over at 4B Satribari Christian Hospital, Guwahati. A fully automated lab was dedicated to service on 17.12.2010. Mr. A. Goldsmith, Executive member of the CBCNEI offered dedication prayer. Mr. Ashoke Kumar Jalan, Chartered Accountant, and a well wisher of the Hospital formally opened the lab.

Now physicians can get fast, accurate and reliable results in Haematology, Biochemistry, Microbiology, Serology, Urine analysis and Parasitology.

We look forward to meet the requirements of all the lab investigations in-house. This is indeed a step towards the making the hospital facilities useful for patients who come to the hospitals. It prevents them the hardship of going elsewhere for lab tests.

Healthcare sector is growing at an unbelievable rate in India, but the cost of healthcare has not reduced significantly. This has lead to a growing population with limited access to healthcare. To reconcile high quality with low costs we believe that a Social Enterprise model is the best.

Despite massive growth in healthcare sector in India, huge gaps still exist in the supply of quality health care to the population at the bottom of the economic pyramid. The size of this population is approximately 4 billion globally. 4B Healthcare gets its name from this particular focus of its services to 4 Billion people, starting from the 750 million in India.
To accomplish its mission, 4B Healthcare uses following strategies:

Evidence based and protocol driven clinical practice

Optimizing input costs in establishing hospital infrastructure

Deploying carefully evaluated technology for its evidence based benefits

Developing a peripheral network of clinics and pharmacies to meet primary healthcare needs

Developing risk pooling mechanisms so that no one is excluded from accessing healthcare

To achieve this, 4B Healthcare is set up as a social enterprise where social impact and social benefit is the bottom line. Financial sustainability is considered critical in sustaining the social goal over a long period of time.
Financing of this enterprise is done by a group of socially conscious investors for whom social returns are more important than very high market returns. They express their commitment to the social goal by forgoing the market return they could have got if it was a purely commercial enterprise. Social return is monitored via specific social performance management programs.

On July 1st 2010 4B Healthcare took over the management of Satribari Christian Hospital, Guwahati. SCH was Established in 1926 by Americal Missionaries as a center for care of women and children. It was the first in the North-East to start training nurses in 1927.
History
Seventy- five years ago American Baptist Missionaries working in Assam felt the need to start Medical work in Guwahati. In those days, social, cultural & religious beliefs prevented women from being treated by men. As there were not many qualified Women doctors in India, and community interventions were almost nil, the death rate among women & children was very high.

As a result of their vision to serve the women in Assam, the foundation stone of Gauhati Women and Children Hospital, a 25 bedded hospital was laid in the year 1923 and in 1926 the first inpatient was admitted. In 1944 the hospital was renamed to American Baptist Mission Hospital and eventually to Satribari Christian Hospital. The late 1970s saw the last of missionaries leaving India and the Baptist Churches taking over the hospital.

Present Status
The Hospital currently has a capacity of 155 beds and not only meets the needs of the local people but also gives services to patients from rural areas surrounding Guwahati and neighboring districts and states.
While many still consider Satribari Christian Hospital an important part of the heritage of the city, it has long lost its place as the cornerstone in the healthcare system of the city. Many reasons may be attributed to this decline, the chief being the inability of the hospital to Keep up with the times, whether in the modernizing of facilities or adapting the services offered according to the changing needs of the city.
4B Healthcare In Satribari
One of the parts of India we have strong links to and love for is the north east.Historically neglected and underdeveloped, but now rapidly growing, the cities in the north-east are unprepared to handle the rise in need in healthcare. The north east also has some o the highest concentration of Mission owned hospitals and religious organizations.
On July 1st 2010 4B Healthcare took over the management of Satribari Christian hospital from the Council of Baptist Churches in North East India (CBCNEI). Over the next years, we aspire to re-establish the hospital as one of the major centers of quality and service in the region.

Whether you are an ailing hospital, an interested investor or a prospective collaborator, what services we offer will give you a clear picture of how we are uniquely equipped for our Mission.

Professional Management of Hospitals

We offer professional management in turning ailing and moribund hospitals into self-sustained and socially responsive ventures. One of the biggest subsets of ailing hospitals are the mission or church run hospitals in India, We have systems tailored specifically to the needs to mission hospitals in helping them achieve financial viability while achieving the original mandate and mission of the hospitals.
Prior to definitive intervention, we start with a situation analysis, market survey and community survey. This helps us answer some fundamental questions such as: what is causing the diseased state of the system, what are the interventions needed, what are the services to be provided, how to manage the various stakeholders, what is the kind of healthcare market in the city or town, what are the existing gaps, how can community be reached and access to healthcare provided, what is the kind of human resource capability available in the area. This helps us put in place either an emergency action plan to cut losses and minimize damage, while we work on a total restructuring of the business model and a management takeover, if applicable.

While the key service we provide is management of hospitals, the change from the charity to commercial is not just about management styles, it is a matter of core beliefs and even worldview. A drastic change in the way we look at healthcare, poverty and money are needed to understand how vital it is that charity based healthcare is replaced, and soon.

Access to Capital

Inability to access capital is a key reason for the run down condition of some of the hospitals with a consequent inability to attract professionals and patients. 4B healthcare network has access to such capital. Though most of this capital come in the form of social investments, they come with considerable discipline and rigor that normally is required when capital investments are used. A robust business plan and a credible team to delver on the results is a pre-requisite for such investments to flow.

Human Resource Development & Management

4B Healthcare uses best practices in the industry to attract and retain professionals. 4B Healthcare is not looking for short term gains but is interested in the professional as a whole including his/her career growth, family, personal development etc.

Connected care

India lacks a credible referral system. Patients with simplest of illnesses end up being treated in a tertiary level hospital thus increasing the cost of treatment. Additionally this also stresses the tertiary hospitals. 4B Healthcare intends to model appropriate primary care that is connected seamlessly to secondary and tertiary levels.

Risk Pooling Innovation

One of 4B Healthcare’s pioneering initiatives is partnering with Micro Finance Institutions in modeling micro health insurance products to help reduce out-of –pocket spending among the poor.
We are also working on finding alternate payment methods for hospitalized patients, who would otherwise borrow money or sell assets to pay for care.

Research

Realizing that these are uncharted waters, we, at 4b realize that all our ventures need supporting by good research and so we are actively involved in designing and implementing studies to evaluate health systems, efficiency of care delivery, accessibility of healthcare and how to make the system universally accessible.
